How to Become a Patient
In order to become a patient in our practice, we must receive an appointment request from your referring provider or primary care provider (PCP). We do not accept self-referrals.

A complete request must include your:
Along with the request, your referring provider or PCP should also send the most recent medical note about your condition and indicate the reason for the request. Most referring providers/PCPs in this area are very familiar with our requirements.
What to Expect
Once you have confirmed an appointment with one of our schedulers, you will receive a text message confirmation from Luma Health, our patient success platform. You will also receive appointment reminders and, depending on your appointment type, links to appointment prep instructions via text message.
Pre-visit Forms
You will receive a text message and email with a link to fill out all new patient paperwork using FormDr, our secure, online platform for collecting patient information. You may follow the link to fill out the forms online, OR you may download and print the forms directly from our website.
You will also receive an email and text message for a Pre-Op Questionnaire from the Green Mountain Surgery Center. Please fill this out prior to your appointment.
