The following forms will need to be filled out prior your first office appointment at Vermont Gastroenterology. You may download and print these forms and either fax them, email them, or bring them with you to your office appointment.

Please note:

The forms should be sent to our office, even if your procedure will take place at Green Mountain Surgery Center.
